A quick question guys. I S-OFF'd, changed my MID and CID to match the European version a month ago to get the 4.2.2 update on my US unlocked model.

To get this 4.3 update I just need to change to the original MID and CID and flash the stock RUU for the US unlocked model and go through the OTAs right? Do I need to change something else?

Yes. Get yourself to 1540.17, make sure you're not rooted, and also have stock recovery, and you'll be fine.

It sounds odd they would not push a finished rom... it's not a different hardware or something like that that needs more work.
