I have noticed that some of the ROM downloads have a .zip.md5sum file. DO I donwload this as well with the ROM and what do I do with it when installing the ROM?

The md5 is only their to check the checksum of the file
so you can tell if it was a good error free download.

CWM and TWRP recoveries all recognize MD5 so you
do not need to do anything special, just flash the rom or
firmware and any other mod that has MD5 as usual.

Even ODIN recognizes MD5 so there is nothing special you need to do.

If you open the file, you will see this:

/home/build/omni/out/target/product/jfltetmo/omni-4.4.1-20131207-jfltetmo-NIGHTLY.zip

The first part is the actual MD5 checksum and the second line tells the program reading the .zip.md5 file to which this MD5 should match/belongs to. Hope that helps.
