HTC HomePlug 5 button

Where can i get this Htc HomePlug 5 button app?



I currently have the 3 button. But i want the 5 button that looks just like that one (with the gray colors)! I only can find the ones that have the color button tabs.

I think I've got the cab you want in my collection:

I'm guessing it's the JUVS or the Kaiser one. But I believe the one in the screenshot have different icons. So install a version of HTC Home and than try to find the icons.
