Problem with keypad

I'm about to go to work so i cant answer any questions till i come back.
So i will try to describe it best i can.

I lost my htc keypad(Phone Dialer) after i installed and removed Quik Menu.
It gave me a wm5 keypad.I dont see any installers for this keypad.I found the files for the htc keypad but i dont know how to make it work.
I installed one of the cabs from this forum to give me the original pad.
But after a few days the wm5 keypad comes back.I have done this 4 times and no matter what it comes back.
Does anyone know where the wm5 keypad sits so i can remove it completely?
And how do i get the stock pad back?
