Default echo on incoming calls

People calling me are telling me that they hear an echo of their own voice. This is a new issue, this has not happened before. I have tried everything I could think of, I have tried three different builds, I have wiped my data and I have tried older .NBH's nothing seems to fix it.

I got rid of it by flashing WinMo, so I know it is not hardware based but I hate WinMo, any suggestions?
