[Q] *228 is broken - How do I fix it?

I just noticed that my *228 doesn't work anymore. I have the Droid Incredible with the following configuration:

Sky Raider 2.52 Sense ROM
2.15 Baseband
CMR 2.5.0.5
S-OFF (Unrevoked Forever)
VZW/Alltel Hybrid PRL 65024

I never tried dialing *228 after doing all these upgrades, and so I'm not exactly sure when I lost my *228 capability. The phone appears to be working just fine otherwise.

But now I dial *228 (options 1 or 2), and the operator just says something like "sorry...no can do."

Is there a way to get this capability back? Or is it gone for good? Do I even need it anymore?
