Uh... I'm going to copy and paste this here since it's really the only answer you are going to get, and to help anyone who might come along later:

If your MEID number is still valid in Settings -> About Phone -> Status -> MEID, then you are probably ok.

You may be able to *228, option 1. If that doesn't do it, try *22899.

If that doesn't work, activate an old phone on your account, then after a few minutes, reactivate your Samsung Fascinate. This will hopefully restore any of the EFS data that wasn't getting pushed by the *228 activation.

If your MEID in the Settings menu doesn't match the one printed on your phone under the battery, then the EFS data was corrupted and unable to rebuild after you cleared it. The only realistic option at that point is having it sent to Samsung.
I have an extra fascinate with a shattered screen. If you have a dead phone I can Frankenstein the screen from, let me know. Thanks!

So if you ever flash something with phone in Odin, you can call Verizon and they can fix it? I can't see myself doing that, but it's good to know, just in case.
My Devices:
RETIRED: Motorola Droid, Samsung Fascinate, Motorola Droid 3, Samsung Galaxy Nexus, Kindle Fire
CURRENT: Samsung Galaxy Note 2, Nexus 7

No! I didnt flash anything in phone section of odin! What I did was flash adryns deodexed/debloated EB01. What I failed to realize was that a box named CLEAR PHONE EFS was checked. Everything worked I just lost ability to use 3G. Gizmos instructions were spot on and worked. The funny part is that I have used Odin multiple times without a prob, so be careful W/Odin. Im not sure what flashing in PHONE section of ODIN would do but I remember gunnermike doing it on purpose to see what would happen and I think he ended up having to get a warranty replacement.

Sent from my SCH-I500 using XDA App

As long as you have the radio file by itself, you can reflash that to fix the issue of flashing something with the "phone" option
