Love Glyphs(Gotham) theme for iOS by Tokems so i make a port for android. All icons are 120x120 pixel. Some icons i got from reddit so the quality is not good. If you have/want to make higher quality let me know
At this time i make icons for apps which installed on my phone only. Please request your icon in this thread only.

#INSTALLATION: Install APK theme file then go to your  launcher's theme setting to apply. Sorry, don't have UI at the moment. Test on Apex launcher only. 
#NEXT UPDATE: Add more icons, Application UI, Icons Request Tool

Works on action launcher 3. Missing a bunch of icons but the ones that it has so far look great. Simple and small.

Reminds me of the Flight icon pack which I love

No. However, you can download and install the apk, then pick the icon pack through the CM12 theme manager. I've tested and confirm that it works [COLOR="Silver"]

You can edit every single icon in nova launcher,by dragging the icon and putting it in EDIT
