Well that is why it is called extreme (I will actually add a warning to that now)

It will be hot because of physics etc etc, but damage to the LED - Yes and no

Yes, being like 5 times stronger than usual, it may wear out the LED more than usual
No, (Kind of) 5 seconds on extreme is equivalent to running the normal flash for say 20 or so minutes, so it will just be like a normal LED, they last ages, so I don't expect that it will burn out before you get a new phone.
