Thanks dzidexx. I think it has more to do with the number of devices, since if I do a factor wipe and start adding devices it gets into the same state.

All, Here is an update, and not sure if this is pointing to a problem with the ROM or hardware but... I did a factory reset via recovery and bluetooth functionality seemed to behave properly. I then did a recovery from google which put the bluetooth back into the same state as it was before...

I rebooted the phone, and now it is there in the notification quick panel and the chop is working. That was driving me crazy. Thanks for the help everyone.

I think I would be ok with the chop not working if I could find a way to turn it on without downloading a 3rd party app.
