Have you tried just starting the game without trying to pre-configure the controls? I started the same way and could not get anything to map so I deleted the .cfg that it created and just started the game. I found that the controls just worked. The only issue I have is that the d-pad...

Just to let you know, Modern Combat 4 works just fine on the SHIELD. You can fire and ironsight with the R1/L1 buttons and sprint/crouch by pushing up or down twice on the left analog. Try the Amazon App store, it's $6.99 there. The game runs very smooth and multiplayer works great as well.
