I had a similar if not the problem the other day. I simply redid the entire process of rsd lite flashing back to Ics then reinstalling bmm.

You'll probably want to format your device back to stock and root again then try applying this again. Check my sig for information on how to get back to stock. Take a look at Step 2 using RSD lite manager.
