I had this problem with omni a long time ago. You can keep trying various roms til you find one that works. or 1) reload stock rom and root it 2) adb shell from computer terminal / cmd prompt 3) - in adb shell: su (or use adb root to start adb as root, you want to see # not $) content query...

have you tried a different computer / linux vm? fastboot has been known to have issues with USB3 ports. do you have a USB2 port to try?

which file do you need? is it not the gpt.bin file in the stock files?
