Try redownloading the recovery (download 4EXT, avoid TWRP at all costs!), and reflashing it. It might be a CRC error.

Meh, it seems to be a lost cause. I had an exact same issue. /system, /data and /cache partitions got completely messed up, with no hope of restoring them. The only possibilities for fixing that seem to be changing the emmc chip... or somehow plugging it under Linux disto and completely...
