Yeah, ditto. I adb shell, su, mount /system as read/write, delete the APKs, verify they're gone, then remount as read-only ... and they're all back!

After a couple more days experimenting, here's what I've discovered: Stock gives me 3G at home and LTE at work. I normally only get the ! no data icon when I'm out in the boonies. I don't think I've ever seen the R icon on stock. On CM12, if I re-flash the APN fix again, I'll get LTE at work...

Already had the APN Change app installed -- it shows same thing as your screenshot. And your file is the same, MD5 hash matches. Thanks for the help. I guess I'll try flashing it again. (Again.) :)
