It also happens when you have the app still in the data/apps folder yet it's already uninstalled. Go to your data/apps folder with Root Explorer and check if the .apk is still there. If it is, delete it.

Check your data/apps folder to see if the apk is still in there even though you uninstalled.

Use the manual root method, seems to work better for most, myself included.
