Great question! Also curious.
Any of your testers have success with stock/stock based roms?

If you run the script , you need not apply the profile from EX_KM

If device has init.d support, then yes it will be applied after a reboot or else emulate init.d from kernel adiutor or run manually

As @Senthil360 said, once the script is applied, the old profile doesn't matter, the name might be still showing on EXKM, but the values are from Project Zhana.

If the profile is made for EXKM, no you need to use EXKM
If the profile uses init.d script, yes you may use KA for apply it
If the profiles uses BASH script or It it's a flasheable .zip, no app is needed for applying the profile. But afterwards you can modify the values with the app you prefer.

I am using shotgut 1.7.6.
In the OP it only has EXKM Profiles.
But there is also one init.d script, is thr script outdated ?

I have to copy the script to /system/su.d/ to have an effect ?

As far as I know Shotgun only has EXKM profile

But other profiles as Nameless or Project Zhana uses init.d scripts

And as you are using Systemless root /su.d folder shouldn't be present and therefore you should be using /init.d folder

I can find both su.d and init.d folders..in which should I copy the profile then?

If you are using init.d emulation, which is the case since we don't have any kernel that supports init.d, I believe that the init.d folder is used.
But try to place it in both, the worst thing that can happen is that the tweak execute 2 times.
