Is your phone heating up ? From what you described it sounds like it's throttling down to prevent overheating

Go through and Titan all the screws on the back of the device I had driven myself crazy with GPS issues trying to figure out what was wrong and it was just a loose connection

You should try running Sensor Box for Android from the play store to check proximity sensor by itself also if it does it when running Stock ROM I don't think there is going to be anything you can flash to fix it I would also make sure that your proximity sensor isn't being blocked by anything...
