So, I stopped doing anything as I got busy with my life. Here comes the 4.1.7 OTA and my DM verity was gone. So, it seems like if you go to stock and if still dm verity exists, try to do ota update or go to one ota back for flashing.

So this is what has happened since I got rid of dm_verity from my 4.0.2 rooted stock OOS state. I updated my twrp to latest (3.1.x-y), wiped everything and flashed stock OOS 4.1.6, locked my bootloader and let it install stock recovery. So basically, I tried to go back to original stock state...

I think OP2 was screed because if Snapdragon 810 which wasn't a stable enough for every phone of that year. Snapdragon 820 on the other end is a totally different beast !

which twrp version are you using? also do you have encryption issues in twrp? you might not need to reflash the rom butmay be reset the phone and install updated twrp. there are couple of threads. i myself saw this issue on my device
