Try to plug your OTG then mount all partitions, after that reboot to recovery without ejecting the OTG. with Miui 8.1.18, I plugged my OTG, I wiped all partition except Boot partition 3 times, I flashed the Rom from OTG without any issue. my Bootloader is from 19s, I didn't use 21s. Do you have an OS on your phone? if yes, are you using any screen lock (Pattern,pin or password)? Try to disable whatever screen lock methods you're using if there is, then reboot to TWRP and check if the problem still the same.

First time I flashed the V4.2, I came from the Stock Rom, it didn't fix the Gyro problem, but as I remember you should delete/rename these files first (sensor_def_qcomdev.conf - sensors_settings - sns.reg) then in TWRP do a factory rest and wipe Dalvik + Cache. Reflash the Rom again.

I only tried (Clean Stock Rom & CM 12.1 V4 by BetaHydri) clean Rom seems have better battery life, CM12.1 has better looking and advanced settings.

I got same problem, in my case I wanted to restore the stock Rom but the phone restart to TWRP before restore ends then I tried to restore the CM12.1 backup, I stuck on LeEco logo. Luckily, after many reboots, the original recovery restored (I don't know how) and I could flash the 11s clean...
