When I was on stock - phone did not ask for the code. :o It only send me to unlock app.

Phone code does not work even if it ask for it. When flash LineageOS rom via TWRP (Marshmallow), phone asks for code, but code not accepted, because it's on custom ROM. Only unlock from professional need, maybe xtc2clip box needed.

If you have AT&T card, just use OTA update. I don't know all things how to update from Lollipop. Plase read Readme thread.
