I went looking for the same thing. I found an app called Catapult which is built from the Trebuchet source. You can find it on Play Store or F-Droid. Seems to run fine. I went so far as to dig the Trebuchet apk out of the latest CM13 nightly and install it as a system app (root file manager)...

Unfortunately the latest version isn't working on my Nexus 5. I can make the change via terminal, but there must be some issue with WiFi still because I'm not able to connect once I change it. The app gives me an error and says the change to /system was reverted. Any ideas?

D820, files from the OP, Windows 8.1 64 bit, Cataclysm ROM (Sep 2015). Good LTE near Munich on band 3. Icon said 4G, but was able to verify connection to band 3 in Nexus FTM app.

On yours, did the Ting entry in the list not have a bubble? I see Ting in the list, but the selection bubble to the right is missing on the last couple of entries, including Ting. Right now the one I have selected is 'internet' which is giving me LTE for the first time since the May 14 nightly...
