It started happening when I recently received the 4.0.2 OTA, stock rom/kernel/radio. Even when turning the screen on and off, the slide to unlock remained non responsive.

So it seems that when I have the standard slide-to-unlock screen lock enabled, the device occasionally ends up in a state where I am unable to slide the circle to unlock it and it appears to become non responsive. Have any others dealt with this issue? If so, has anyone found a resolution? I'm...

It always is and always will be user preference. Some people have lower thresholds for the little annoying things. Getting it replaced is solely up to you. For me, it's not a big enough issue.
