No joy. I fastboot flashed the TWRP image. I then went to manuallly change the name of the file. First I had to mount the system partition. Then I looked in /system and saw that the file was already named "recovery-from-boot.bak" so there was nothing to rename. Any other ideas?

I've always been able to fix this by going to Developer Options (assuming you've set yourself up) under Settings, selecting "Select USB Configuration" and then toggling from MTP to PTP and then back to MTP.

No luck. I attached the Nexus 4 to the computer, entered a command prompt to navigate to the platform tools director that has the adb files and entered the "adb devices" command. The response was "List of devices attached" with no phone (or anything else) listed.
