I would suggest that you raise your min cpu to 384. That is how HTC has it and that has been less problematic for me.

I would try raising your min cpu to 384. At one point that actually stopped reboots for a lot of people, my self included.
Will try, even though I think that wasn't the problem of my reboot as the frequency was set to 540MHz if I am not mistaken. ([136782.623779] [K] CPU0: msm_pm_power_collapse: restore clock rate to 540000)
Your max screen off is set to that or your minimum? Having the minimum set to 192 has caused issues like that, raising the max screen off helps with apps like Pandora or music that run when your screen turns off.
