this temporary solved my all problems.....works on both all S4 Versions i950.........

<permission name="android.permission.READ_EXTERNAL_STORAGE" >
<group gid="sdcard_r" />
<group gid="media_rw" />
</permission>

<permission name="android.permission.WRITE_EXTERNAL_STORAGE" >
<group gid="sdcard_r" />
<group gid="sdcard_rw" />
<group gid="media_rw" />
</permission>

<permission name="android.permission.ACCESS_ALL_EXTERNAL_STORA GE" >
<group gid="sdcard_r" />
<group gid="sdcard_rw" />
<group gid="sdcard_all" />
</permission>

or download the file and flash it via recovery
plus set 744 permission on folder mnt/media_rw [on every reboot]
Using root explorer manually

@henrik.dk I made the switch to Titanium Backup and it doesn't require changing permissions on the media_rw directory, only the modifying platform.xml, so it works fine after rebooting

this fix ,both for read and write external storage has to be manually modded even on official 9500 rom... wth samsung and google!!! didnt expected this in an official rom

If only someone was capable of creating an Xposed module that would change those rights automatically everytime the phone restarts!! That would be heaven

i installed with method 1
but now i had many FC's, ican't open any app and i can't do anything in my phone

what's happen !!!!

is there a fix ???

I had a same error like this, but after several reboot times, everything's ok. maybe you should try. I didn't have any problem with my gallery.
