You have to follow steps 2 - 4. HTC emails it to you...
I would like to add for those that like my toolkit, PLEASE DO NOT POST IT ELSEWHERE WITHOUT MY PERMISSION!
I have seen multiple sites who copy and paste my work to their site! If I do not approve of it, then do not do it! it will discourage me from adding updates since it is being used for their own benefits!
tl;dr.
If you post my work without permission, don't expect updates!
Unfortunately, that is the internet... lol
Don't expect to not find your stuff re-blogged and errors in their posts. There are tons of blogs that "copied" my root methods for the Hero and EVO and my other programs/threads, and they contain errors that I didn't write as well. All you can do is keep going with internal motivation. That's what keeps me going at least! Once it turns into a competition for recognition/money (not saying that you are trying to with your post, just in general) here on XDA, the dynamics of the whole site start to change.
That said, you did choose to write this using the .NET framework and I know people will decompile it and use your ideas in their own projects unfortunately. That's just the way it is out there. I just publish my source on Github and let the world learn from it. This would be much different if you were charging for your toolkit obviously.
Ah lol I must have had your project mixed up with another.. Obviously it's not written on the .NET framework. But don't be surprised if you still find other people posting your stuff. There definitely are great people out there that will ask for permission, but you know there are always ones that wont.
If you flashed it once, let the phone load up and press the button again.

Having a little problem trying to flash kernels on the unofficial official ics update from tmobile. Wanted to put root, everything worked in the sense of unlocking, but once I messed around with the kernel I can't get passed the HTC screen.
Flashed kernel twice liked you posted, still nothing.
