I used 4.4.4 stock and modular on the newest cm 11. All hashes verified and did formats before installs.

Yes, stock camera app in video capture mode. I have also tried another app named 'A Better Camera'. I haven't had a chance to try the stock Samsung media_profiles.xml that lives in /system/etc.
I used the Paranoid Android gapps 4.4.3 package named pa_gapps-stock-4.4.3-20140608-signed.zip. Is that what you used Daniel?
