This seems to be a software issue, as I have it happen all the time on my rooted 5.0.1 OTA updated S4. It never happened before the OTA, and even after completely wiping the phone with ODIN, the problem has persisted. The 5.0.1 update on the Verizon S4 is very, very buggy.
