Do that:

File explorer-->My Device--->Application Data--->HTC--->Progluncher

Delete all files in that folder and softreset. That should solve the Problem!

Thanks, this helped me out for most of the icons.
Unfortunatly the Icon for JBLEND (Java) still doesn't want to change.
Is there anything else which I need to clean up?

Does no one has any idea why it isn't working for some Hometab Icons?
I noticed it is also not working for QuickGPS. (Only on the hometab)

I guess no one has a solution for this..

I removed the entry of MS Office in the registry:
[HKEY_LOCAL_MACHINE]\software\security\shell\startinfo\start\office or [HKEY_LOCAL_MACHINE]\Security\Shell\StartInfo\Start (I dont remember exactly)

Reinstalled office again and then I'm able to change.
***But if you are working with a rom which comes with office, then don't try this. ***

Try to verify if in the registry there are more than one office instance.

When you edit the start menu folder, the registry is automatically updated, but not for some cases, then you need to fix the registry to match with the start menu.

Update!!
I've found a bug/feature in manila which prevents some icons to be changed.
Since many users were complaining about not being able to change some icons I've started investigating the problem and found that if you try to use a custom icon which have the same filename of one of the images found in \Windows then Manila will always load that instead of your customized one.

In order to easily fix the problem, just rename your icon :)

This bug affects ONLY manila, therefore in the start menu everything will look fine BUT in the Homescreen you will always have the original icon!]

Thanks for investigating, my phone looks all right now! :D
I did my own investigation, but no luck (even replacing the file in /Windows didn't work for some reason) :)

First my english is very bad, please forgive.

The registry contains for the most icons a backup path to the old icon. Delete the path or edit the path with the new icon.

Also you can change icons in the registry, which does not know csmi.

In my ROM is the Path HKLM/Security/shell/StartInfo/Start
