I cannot get it to install on 4.0.3, Can someone confirm this or is it just me? Tried to install while using Sense and also Apex launcher. Both didn't work.

To which version of Evo is this apk, GSM or CDMA? In GSM Evo ICS can not be installed.
This application runs fine on GSM mwakious Rom.

It should work fine on sense and on both CDMA and GSM , try putting it in system/app and setting correct permissions .

@op thanks for the app. Here's a signed copy of the app you posted. This one you can install as a regular apk. No need to push to system or use root explorer. Works on sense and should on aosp, aokp,Cm7,Cm9 etc.
