Can't use new MicroSD Card

I have a 16GB microsd that works no problem, but I'm trying to upgrade the storage size.
I tried pretty much everything to get this new microSD card working, but nothing works.
I tried both a Sandisk and a samsung, they work on my girlfriend's phone no problem (LG G3).
I can't mount it, it tells me that the SD Card needs to be formatted. It won't format on the phone, and I tried formatting in windows several times but nothing works.
I can't help but feel this phone is defective, or is there some special way I should be doing it?

Format the card to NTFS then inset into your phone. It should work. If not, format the card to FAT32 then do the same thing. There's no real reason this should be happening to you

Same issue, just won't work.
