I was getting this and I found it was due to holding down the Volume Up & Power Buttons for too long.
Forgive my ignorance, but what is the button sequence to hold when booting to SD? I didn't see it listed so I guess I thought it would just recognize the SD and boot from it.

I was powering off with volume up + power, then powering back on with volume up + power, then releasing both buttons as soon as I saw the Nook logo.

I am having a hard time getting it to boot in CWM from the SD. I've tried three different SD variations including one that worked on my Nook Color.

I've tried using both WinImage and WinDiskImager to write the SD.

After I power up it I'll get a choppy looking title which I cannot read at the top for just a second and then the screen goes blank and it just hangs.

I've read leapinar's helpful tips in his FAQ (A 9. Bootable SD Cards) but nothing seems to be working.

Any helpful tips would be appreciated. Thank you!
Are you using the right version for your device? HD or HD+?

Edit: That is not the problem. I have both devices and the cards will not boot at all on the wrong device. The only other thing I can suggest is another card. I know you said it worked on the nook color, but it still may not be reading the CM10 files correctly.

Just to clarify, at this point I'm just trying to get CWM to boot right now so I can root, but yeah I've got the right HD files for my device though I tried the HD+ too just out of curiosity and when I used the HD+ it hesitated then booted into B&N stock.

So far I've tried, unsuccessfully, a Samsung 8GB class 6, a SanDisk 4GB class 4, an unmarked brand 8GB class 4, and with the HD+ file a Samsung 2GB class 4.

I might have a few more at work I could try, otherwise I guess I'll have to buy one of the known working ones.

On a side note, leapinlar, thanks for your hard work on this!

Since I posted the new CWM that works on the HD, those zips that are marked HD-HDplus will work on either system and will definitely root your HD. I did it on my 2.0.5 HD.

Seeing as everyone has done so much good work on this I thought I would post a little tip I found, it maybe of use to some one?
My thing was the google drive app, that is all I wanted on the device and thus that is why I rooted and installed gapps.
I found that once I installed the google drive app, everytime I went to the google drive url on the browser it offered to load the app instead, so I bookmarked the google drive website and added it to my nook desktop. Now when ever I click the website bookmark link on the nook desktop it takes me to the app instead, without needing to go the the alt launçher or 'hold vol up and press apps' method.
I supose if you can find a url to launch your own apps you will be able to add nook desktop shortcuts to apps too?

That makes the Nook boot to CWM from the SD card.

What worked for me was:

Downloaded NookHDplus-bootable-CWM-6023-for-emmc-stock-4GB-rev1.zip
Extracted the .img file
Wrote the .img file to a Sandisk 8gb Microsdhc Class 2 card & put the card in the Nook
Powered the Nook off with volume up + power & released buttons when it was off
Powered the Nook on with volume up + power & released buttons as soon as I saw the Nook boot logo, it then booted into CWM from the SD card
Flashed NookHD-HDplus-Universal-Root.zip from the SD card using CWM
Flashed NookHD-HDplus-Gapps+Extras-for-stock.zip from the SD card using CWM
Powered the Nook off with volume up + power & released buttons when it was off
Powered the Nook back on, booted into stock ROM & Google play was ready to go :)

Big thanks to leapinlar for this, as he has turned what was a nearly useless tablet into something perfectly useable. :D

Apparently, I am not up and working. Before re-rooting I was recharging my HD+ and did so overnight. The next day as I was preparing to re-root it after the 2.05 update, I noticed it had only gotten to 28% via the AC power charger. I tried charging it on a PC but it discharges instead of doing a recharge. I can only get 28% battery life if I recharge with the AC adapter. Any solutions to this issue preferable without having to reset to factory?

You can get the CWMs attached below. I have one for HDplus and one for HD. You must use the version for your device, they are not interchangeable. Download the appropriate CWM image zip and extract it. You will end up with an approximate 3.7GB .img file. Burn that image to your 4GB or larger SD using Win32diskimager in Windows or use the dd command in Linux or OSX.
I'm having trouble with this step in the flashing. Do I have to do this just to root? If so, can someone give us more explanation?

You mean you are having trouble burning the image to an SD? Yes, you must make the bootable cwm card before you can install (flash) the root zip file to your nook. It is the tool that allows you to get the zip installed. Go to my nook color tips thread linked in my signature and look at item A9. It describes how to burn the image to your SD. It was written for the nook color, but applies to the Nook HD/HD+.

I am back with an update.

I've been using this now for several days and the one problem I have is that I cannot set Gmail to sync automatically. Every time I try, I get the 'stopped unexpectedly' error message. Not a big deal, I can sync it when I start it of course and it does that, but I can't set a schedule without it crashing.

Other than that, everything runs great, it's exactly what I'd hoped for and you all who worked so hard on this are brilliant!

AWESOME. I am so in this game now. I successfully burned and rooted and right now I am making a nandroid backup. However, how do I now get back to my Nook? Do I have to take out the SD card?

Install AnyCut which will allow you to put the Nook Homescreen shortcut in your taskbar or main screen. I'm using Go Launcher as my launcher app of choice and have the Homescreen shortcut in my launcher bar.

The Homescreen shortcut takes you to the Nook OS homescreen, the N takes you back to whatever launcher screen you're using.

What do you mean recovery? It doesn't go through the 8 bootloop sequence, does it?

And take the card you burned out.

Mine boots into the new Android style OS and I'm using Go Launcher, so when it boots, I get the same screen set up as on my Photon. Then I installed Anycut, put the Nook Operating System Short Cut on my home page and when I click that shortcut, everything goes back to the Nook system.

If you installed everything correctly, that's the way it should work. If it's doing something else, then leapinlar or one of the other devs will have to walk you through it.

Use a Sandisk class 4, class 6 or ultra class10. All other brands are a gamble that it may/may not work.

Yes, take out the sd card and select reboot.

Ok, I just hit the power button without the volume up button, and it booted to CWM just fine. Then when I was done flashing the zips, I removed the SD card and selected reboot from the CWM menu.
