I can't use your kernel with cm12.1 from @dgadelha http://forum.xda-developers.com/showthread.php?t=3299201
Clean cache and dalvik after install, works with little lag at first boot, then freeze at next boot. Maybe dt2w problem.

hey mate, you should try to use anykernel.zip fir making flashable zip, this will reduce your work, you can have a look at my build script. This will allow you to flash the kernel the on cm 12.1 based rom with no issues with ram disk
