I actually have same problem with my win7 OS and NCS that doesn't recognize my dead-lumia 800 plug-insertion and ask me to repeat the procedure. Have you solved in your OS that ask you to format the disk? Or it's the same?
May be an USB driver problem wich dosn't recognize the phone?
