maybe connection problem between pc device
update your drivers (Qualcomm 9008)
try another usb port
it would be good to disable the driver signature (before than all)...
everything is explained in OP

Ok, I have dubble checked USB ports, cables, tried all the drivers listed in OP, disable the driver signature and still the same.
Is there a way to make the MDT log to file?
(I also guess that "Downloading vendor.img to vendor means writing the vendor.img to the phone.)
Also the Led on the phone is green all the time, like it is connected.

Any more ideas?

Don't press VolUp+Power. Disconnect your device from PC. Shutdown your device, it may seem off but it usually isn't in this process(I did it by going to bootloader and used the option Power off). Then just keep the volume up button pressed(no power key) and connect to your pc.

It worked for me. My phone crashed last night and had to perform the entire hard reset

That brings me back to the same thing, stop working at the same place downloading vendor.img at 349sec
