It appears that the unofficial CM13 Marshmallow is out there and is stable. Will there be a guide on how to install that as well?
Thank you so much for this Tutorial. I have been putting off doing a perm root and was booting CM10 off an sdcard. After reading this I went for it. It was easy.
However, it seems that CWM and the latest CM12 roms don't get along. I had to boot into TWRP instead, then everything was smooth sailing after that.
I followed the guide, with the above changes. Then ran a backup of CM12. Then Wiped, and Installed CM13 and the appropriate gapps. Viola.
Works like a charm. Only problem I have is I can't get TiBU to see CM13 as rooted.

Cm13 worth the upgrade from 12.1?

Is it worth the upgrade? And sorry to sound dumb, but I want to know how to flash CM13 through TWRP? My nook's currently running 12.1. Thank you. I'm not much of a techie so I'm having a hard time understanding jargons used here :(

Sorry I can't help with this decision; haven't yet tried it myself.

I would expect no problem using one of @amaces releases, providing you can find a suitable GApps package to go with it. The open-gapps provider has added a hint to the Android 6.0 package about a needed ROM patch. I haven't yet figured out what this actually means.

Please let us know if you try, and how it works out.

There is a cm13 thread in the Development Forum. I have installed it and it works well. The only complaint I have is Titanium Backup will not recognize root. Not sure if that is a rom or APP issue

What GApps package did you use? If open-gapps, could you please tell us what is meant by the "ROM patch" hint that it presents?
I used CM14 from the Notre Dame filebox link in this tutorial and then gapps from opengapps, no patch, works fine. If you go to their site, just pick ARM, 6.0 for CM14, and then you can pick which package. I think I did micro or nano. I was starting from stock os though, not upgrading from previous CM version.

One other question...
So this is my first time running CM. If I try to watch something on Google Play Movies, I get an error saying it failed to fetch certificate. Is this a CM or root issue? I know at least at one point in time Google Pay didn't (maybe still doesn') work with root, and I think Google Play Movies didn't either, but I thought at least the movie app was updated to work with root. My Moto X DE has the bootloader unlocked and su is active and Play Movies works fine with that. I thought it might be my wifi at home, so I switched the phone to wifi and it still worked.
Any have any thoughts?

Also, YouTube doesn't work. I also tried tethering to my phone and it didn't help. Additionally, I tried to install twrp manager and it stays I don't have root. I tried to install both SuperSU and the old superuser. SuperSU says it can't update binaries, old superuser just crashes. Is there a setting in cyanogenmod I need to change?

Thanks for this guide. When I followed this guide a few weeks ago though, the only available package I could see on the links was actually marshmallow and cynaogen 13. I used this instead and have found a few bugs, but I am not sure if these are related to cynaogen 13 generally, or cynaogen 13 with the Nook HD+.

1: Bluetooth does not work. It will not pair with anything and just hang. EDIT: Scrap that. Only seems to not pair with my car stereo. No other device has issues with it though.
2: Sometimes it goes into a mode where if you swipe to unlock, it will unlock but then the screen goes instantly black. Then you are back to trying to unlock it again and the loop continues.
3: Google search bar is not drawn correctly on home screen. Some of it is cut off.
4: When you shutdown, more often than not, the Nook restarts straight away and will not stay powered down.
5: A weird bug, where the screen keeps flickering bright then less bright like something is wrong with the brightness settings and it fluctuates.

Do any of these issues occur for anyone else? Are they not present on 12.1 or 11?

Are you using the 02/08/2016 build of CM13? I had the same problems you are having on a previous version, but once I updated to the latest version they went away. So far it has been almost flawless for me
