Japan OTA must have an extra obscure URL because a Community Manager at community.sony.com says he contacted the Sony Japan Engineer team and they said 4.0.3r5a has been released for Japan region. CLICK HERE

OK got the new update and rooted it before upgrading my tablet but after reboot, the bloody thing revert back to old firmware.
Already wipe and format it but still the same thing happen.

What need to be done?
I remember that I need to press "vol" + "power" before the 3rd green dot appear while installing the custom firmware to wipe the data?

Already press and hold the Vol+ when the 3rd green button light up but still get the old firmware when reboot!

?? Don't get what you're trying to say here.
Oh, please see here
Condi already said that.
If after wipe or reboot your device will boot to old firmware (which is still on 2nd /system partition),
then you should use v2 doubleFLASH custom zip - which updates both /system0&1 partitions when flashing
This prevents from such problems!

Okay!

Okay!
That custom firmware is for SG (Singapore) tab S?

The Poland ROM can solve all the errors(error 7, caused by root).
Because the Poland ROM's base sku is completed.
I faced the problem of error 7, and solved with Poland, although mine is Japan.
HERE
Hmmmm ... hard to understand if you really understand my problem .
Mine tab is rooted under old version. Make a rooted firmware for R5A and install successfully but every reboot will revert the firmware back to the old rooted version.

Finding a way to make R5A stick is my problem and I prefer using my region firmware.

Yes, I see.
Are you installed the "doubleFLASH" of Condi?
