The settings take effect immediately.So Installed this last night. In the app, do I have to turn these features on using the blue power buttons?? Or are these features I selected during setup automatically on? Also swipe to sleep, how the hell does that work?

Sweep2sleep is activated by sweeping alng the edge of the screen, where the nav buutons are.

Just dirty flash the new kernel version and you should be ok, no need to do anything else, what you have is exactly the same as what I have, just pay attention when the installer gets to SElinux part, in my case I chose permissive in order to install V4A. No need to re-root either, SuperSU survives.If I am already on the stock ROM running v4.0.3 (flashed TWRP, then v4.0.3, then SuperSU v2.5.2 to get root) is it fine to flash 4.0.4 over 4.0.3, or should i re-flash to stock factory image and then flash TWRP the kernel and re-root?

You don't need to reflash /system
The only concern is if you are using the latest stock (MRA58N) and you flashed Chainfire's boot.img, then you should flash the factory boot.img. Chainfire's boot.img uses MRA58K. Not a big deal, but might as well have everything up to date.

There were no changes to the kernel that would have done that.Flashed 4.04 today, made my nexus 9 laggy. Reverted back to 4.03 and its back to normal.

Well OK I did not bought your app .... but, I would have think more people would be curious to know!
To report back after a full 24h of use : everything works nicely. Well, not as smooth as with default settings, but I have had absolutely no notable lags, just few shutter.
The best part is that I have 4h of SOT (browsing, email, video, kiosque, ...) and still have 43% left of battery. For the same SOT with default settings and a similar usage I had 10% left yesterday!
This is a freaking 33% difference!
