Network Unlock Problem

Problem :

I am entering code it says OK ! then restarts but asks again. Then i try 00000000 or 11111111 it accepts all codes. But after restart asks code again.

Tried :

* Format
* Install Stock Ruu.
* s-off unlock
* s-on locked.

Still same. Any ideas ? Or any thing to unlock with software ?
