Seems like phone's hardware malfunction. Try to go to TWRP recovery and flash ROM and gapps again. This time do a factory reset before flashing. If you face similar problem go back to your previous ROM and test.
I had this also once with V3 after 3 days without booting. Phone froze I had to press power button for 10 sec to force reboot.....

But now with V4 the standby battery is really great:

As I didn't change any other bits on my phone. It could only be phone settings specific or OTG cable problem. Try also to use a USB Hub in between. BTW if I plug in a USB stick the mount USB settings are grayed out, so it works independent of this option menu inside the phone setting.

Question: Are all other users of my ROM having the same OTG issues?

I did not testet otg, but i got freezed screen in v4 too, v3 without any freeze. Only one thing in v3 i do not like - how can i rid off flashing diode by incoming call or sms?
Installed without factory reset.

Edit:
So usb otg work, try install es file explorer first.

with titanium backup or adb or some other root app uninstaller remove the app called flash ring.

So usb otg work, try install es file explorer first.
Maybe too much tuning inside the build.prop ?. Dalvik cache for instant and power management settings was tuned by me. If it freezes often, play with build.prop or interchange it again with V3.

I'm having issues with Settings crashing when i enter Apps submenu. I can't unninstall or do anything from Settings>Apps menu(i have to use a 3rd party app). I had to reinstall v4 and do a fresh setup of my device, since i had alot of crashes with my twrp data backup. Any idea on what's going on?

Change it with the from v3 build.prop and let me know if it's better for you. Have you the 32gb version of the 1 Pro?

I'm using V3 build.prop with all changes from V4 except LTE and UMTS settings.. The LTE changes were causing me FC's but so far 24 hours later no FC's.
Could someone wth OTG test this build.prop?

Can you identify which line in v4 makes it crash?
I have the 64gb US version.

yes the one in the Zip. There were some changes to usb in v3 to v4 look at usb near bottom of build.prop there are differences. I also removed all the #'s from the Better internet browsing & download speed tweaks not sure about that but it didn't look right in V4. The power tweaks are still there and for the first time since I've had the phone was able to play games on charge and it charged well and didn't get hot .

OK, but I noticed that not all power saving settings weren't in that build.prop.
I changed now the telephony default network back to 22,22 and default 10. And I also increased the dalvik cache. I'm updating the V4 ROM with that changes....
I hope that the root cause is not the power saving !

Your right I missed those..I'll fix that and test it... but just could not change telephony back 22,22 and change back to 10 it still caused FC's . only way to stop FC's was completely remove all changes to LTE

I've addes all those line on this build.prop and it's still good. I guess only the LTE settings was making settings crash.
