Phone randomly changing settings when on call

When I am on a phone call, my phone will just randomly change settings. When the phone is up to my ear, the screen goes off. However, I will notice most of the time that Bluetooth, GPS, and screen rotation have been turned on. Sometimes, my phone will open notifications, and the menu will be down when I look at the screen again. Is the screen just failing to disable the touch input?

I am completely stock and unrooted.

Sounds like a hardware issue possibly.
