Ok this is my problem.

I'll try to use either CWM Manager or ext4 Manager it will go to reboot my phone to perform a task (like convert a filesystem or install a .zip file).
When the phone reboots, it does go into recovery, but it won't perform the action that is supposed to happen.

Is there any files that are stored on the phone for CWM to automatically install a zip on reboot or something like that? Maybe there's something there that's preventing the operation from happening.

Any help would be appreciated.

BTW, its been working fine for me for ages, its just now that this has happened.

Problem solved. In my case the cache partition must be have corrupted. I just used ODIN to re-partition to reflash and all is fine again :)
