Check whether module /system/lib/modules/dhd.ko exists, or what's better has /system enough space for modules.

Latest kernel is R2.22.2, check main topic in development forum.
1. yes you can. I am afraid there's nothing to describe since it only requires simple flashing.
2. AFAIK it doesn't support it. At least you can;t use it on /system. If you want, follow instruction in topic, to check whether it's possible to use it on /data and /cache.
3. I guess you're using too old kernel version.

Just install kernel over existing installation or just extract dhd.ko and manually put in /system/lib/modules.

Are you sure that you're installing correct version? As status 7 error happens to prevent incompatible version being installed.

I'm using GAPPS from original thread polishvodka
@forum.android.com.pl

Is this problem can be related to CWM reparted .. ?
I'm using cwm reparted system 650 and 620 both have same result ..
I'm checking system partition with diskinfo
- without GAPPS system partition left around 23 M left
- with GAPPS there's no space left

If this problem related to system partition ... Would u make cwm reparted with recommended size for CM12.1 .. And provide a link for it...

Thankyou for ur time n efforts
Cheers....

Try these gapps, http://www.andromods.com/download-files/android-5-1-cm12-1-gapps-full-lite-minimal.html
Otherwise ask in CWM reparted topic for installer that extends space on system more than previous ones(620 and 650M), I don't have time right now for this.

hi chrono..
thanx for ur hard work

I'm on cm12 polishvodka f2fs latest build with 2.24.2 chrono kernel..
I like to ask u.. what specific step to flash ur latest kernel without reflash
from beginning...

every time I'm flash ur kernel I have to do this step :
flash MB1 stock ROM -> Flash latest stock ROM -> flash ur ROM -> flash cwm reparted -> flash cm 12 -> flash ur latest kernel..
if I'm doing straight flash (wipe cache and dalvik) ur ROM I'll get .. error7

my question is, is there a way to cut this step.... to make it shorter step..

thank you

    >>Instruction: run avs_generator.sh, find /sdcard/liveopp_avs, then run it.

Did I understand correctly, if I reboot my phone, I need to re-run liveopp_avs?

Yes - Try it for a day or more. If the values are stable you can move it in systen/etc/init.d (and set correct rights).
Then the phone loads the values at system start...

Is r3.1 version working on Omni LP 5.1.1? I can't run your kernel on this ROM since version 2.24.7 (included).

Yes and not. Uploaded r3.1 package doesn't contain needed ramdisk for OmniROM 5.1.1, but ROM package - does. Extract osfiles/omni_5.1.1.cpio.gz from ROM zip, put in kernel installer with replacing osfiles/5.1.x/5.1.x.cpio.gz and flash kernel.
