The original issue had nothing to do with the install button not appearing. Please post on the correct thread.
As for the original problem it seems to be the Amazon store having issues with the new permissions system (or something of that nature) on the external SD card. I have encountered it several times. The only work around that I've found to work with every rom and every time is to unmount the external memory card, then install whatever app it is I want from the Amazon store. Then remount the card or restart your device. Is something that Google and Amazon need to work on. Hope this helps.

I had the same issue. Cancel/Install screen popped up, but no matter what I tried, the Install button would not be selected. Simple solution, at least for me. Check to see if you have any type of screen dimmer, whether as a separate app or bundled in one. Make sure it's disabled. Viola! Worked for me.
