[Request] Smoked Glass theme for Incredible CM6

I absolutely LOVED the Smoked Glass theme on Droid when I owned it and so far I have not seen it for Incredible yet... I don't care so much for the app drawer as we have great launchers, but would love the same notification bar and dialer.

Can anyone either port it or make one like that? ... I really WANT this look:

Awesome, thanks touchpro!!! Does this have the same dial pad as in the images in OP? (I really love those)

Also, is this only for CM, or can I use it for other AOSP ROMs?
