Like many on here, I was on a custom ROM and had no backup of the stock T-Mobile ROM.
Additionally, the OTA does not like the fact that you have flashed a custom 2.1 or 2.51 .SBF boot/devtree file.
I managed to successfully restore back to the official T-Mobile froyo.
The main difficulty was removing the asserts in the updater-script files in the 6.19 and 4.49 files from T-Mobile.
Removed: Asserts for UMTS_JORDAN and for the MTD:boot:.
I used Autosign to re-sign the modified update.zip files so they would pass signature verification.
Important note: Name the files something OTHER than update.zip ! I found that clockworkmod was caching update.zip, and causing problems.
Select "update from zip" -- do not select "sdcard: update.zip"!!
Here's an attempt at a walk-through!
(Note: Walk-through not entirely for n00bs! Please see any other thread regarding clockworkmod, nandroid backups, rooting your phone, etc., for help on these subjects if you do not understand anything below)
Files you will need:

Steps!
1) Get yourself rooted, and install tenfar's clockworkmod 2.5.1.8.
2) Restore the nandroid backup of the stock t-mobile eclair
3) Using RSD Lite, install the 2.51 boot/devtree .SBF
3) Root, reinstall tenfar's clockworkmod
4) Put the modified .zip update files on your sdcard
5) boot into clockworkmod recovery.
6) Install from zip file the updated eclair .zip file.
7) When it's done, do not leave the bootloader. Immediately install the updated froyo .zip file.
8) When it's done, wipe data/cache, etc.
9) Reboot into the bootloader. (hold power and volume up key)
10) Flash with RSD-Lite the boot/devtree file from adlx.
Reboot!
