Not sure why you're having issues, its pretty strait forward.
Flash the 39a bootstack
Reboot out of the recovery and back in.
Clean flash the ROM, and you're done.
Since you're on the 39a bootstack the only ROMs you'll be able to flash aside from this, is jpcs cm12, or any of the other stock 5.0 ROMs.
To go back to an older version of this, or any ROM that uses loki., you need to flash the 26a bootstack, with the 12b bootloader
I found all the 12B bootstack, modems and the build from 2/2 ... but now my phone reports, that it can't find a sim card ... I used the gapps from 3/18, could this be the issue? I wouldn't think so.
I will go back and see if I can start the 2/2 ROM without gapps ... but so far it seems I don't get data service back on my phone.
Is it possible that my SIM card went bad and causes all these issues?

Looks like I'm not getting this back to a working phone... next I try to see if I get it reverted to stock again...
So I don't know if there are any settings I have to pay attention to, but in Settings / About / Status it looks like the Mobile Network State reports Disconnected.

Trying to talk to Verizon to see if something on their end is preventing the phone from getting into mobile data...
If you never made a back up, or wiped it, there is no way to restore it. That's why its imperative to always have a back up, incase your iemi gets screwed up.
Check your folders there should be a back up from when/if you used ioroot to root your device.
Less you wiped internal.

Well if you don't have a efs folder then I'd say you didn't use ioroot to root it, or deleted it.
And yeah the iemi will be connected to it, but you can't put it back on the phone if its gone I don't think.
You could always try using the lg flash tool to reinstall compete stock as a last ditch effort.
safe to say that this rom support is being dropped for the new one that was just posted up?
Amit posted that the guy making CR droid is quitting it. So yeah this is the last build less someone else picks it up.

Make a backup of your efs this time. If you have data issues or it shows no iemi after flashing a ROM, flash the zip you get from this.
Can never be too safe. I have a copy on my phone, one in my email, and one on my PC at my house.
Flashing radios and stuff is what jacks it up
