On CM12.1 someone reported that it "survived" Youtube updates.

Install Latest Youtube App ,Login With Your Google Account
Open it and see any video For A Minute..

Now Open Your File Manager(ES File Explorer) & Navigate to /data/data/com.google.android.youtube/shared_prefs (use any explorer with root permission)
Open youtube.xml

Find:

<boolean name="h264_main_profile_supported6.0.1" value="false" />

Replace to:

<boolean name="h264_main_profile_supported6.0.1" value="true" />

Save the file ..

That's It Now Close the YouTube App & Open it Again..

Try This May Be This Will Work On CM13 Too..

Fix for com.android.phone stops after call.... (Phone FC)

1) Download the file given in attachment.
2) Copy and paste it in /system/framework folder using ES File Manager (Give Root permissions to ES File Manager)
3) When it asks to overwrite... do the same...
4) Change its permissions to rw- r-- r--
5) Reboot phone once.

Single SIM users on dual SIM device in CM13/MM based roms, you can just force enable true single SIM mode in cm13 using a build.prop edit and 3G to 2G switch will work.

Fix :
find this line in build.prop and put # in front of it
persist.radio.multisim.config=dsds
