Make sure you are running one of the latest recoveries by ADC-Team ! You can download from here
If coming from a non KitKat ROM:
*Factory Reset (wipe /data /cache and dalvik-cache)
*Format system
*Flash ROM and then Gapps
*Reboot
If coming from an older Carbon build:
*Wipe /cache and dalvik-cache
*Flash ROM and Gapps
*Reboot
